Advance directives are known by numerous names. Previously in Minnesota, there were several types of directives, including durable health care powers of attorney, living wills and mental health directives. The law was changed in 1998 and since then there has been one form, the health care directive, for all future health care instructions.
Health care directives can be drawn in general terms or in great detail, specifying:
- The person or people you want to make decisions for you
- if you are unable to communicate, or
- if you wish to have someone else decide for you
- Where you want to receive care
- Your instructions regarding artificial nutrition and hydration
- Your instructions regarding donation of eyes, tissues and organs
- Your instructions regarding funeral arrangements
- What to do if you are pregnant
- What type of care you do not want to receive
